This survey instrument is available for research and public health use, as described in the research article authored by JE Ebinger et al. Symptomatology Following mRNA Vaccination Against SARS-CoV-2. References within the survey to the vaccine dose number administered (e.g. first, second, third) may be adapted as needed.
